Case Study of stroke patient at Patna Centre

Mr Sanjeev Bajaj, a 50 year old had a stroke on 24 July 2019 (right hemiparesis). He came to IHIF Centre in Nov 2019 with complaint of acute onset weakness of the right side of his body along with slurring of speech. His past medical history includes Hypertension and Type 2 Diabetes Mellitus. On assessment, he was found to have decreased muscle strength in his right upper and lower limbs. The tactile sensation as well as the proprioception & kinaesthesia were severely impaired in his right upper and lower limbs. He required assistance of one person for personal care activities. He had reduced dynamic balance requiring standby assistance of one person for mobilising indoors. His gait was circumductory as he had limited ground clearance due to weak hip flexors and ankle dorsiflexors.
A customised treatment plan was formulated for him which included Pablo, Treadmill with harness system, Functional electrical stimulation, Mirror Therapy, sensory re-education, proprioceptive neuromuscular facilitation, gait training and manual exercises.
Currently, he has made very good progress in his neurological rehabilitation program. His general activity and exercise tolerance has improved. His static and dynamic balance has improved greatly enabling him to mobilise indoors and outdoors with standby supervision. The muscle strength in the right upper limb has also improved resulting in him requiring only minimal assistance to complete his personal chores. Although improving, the sensory impairment in his right hand impacts his ability to use his right hand for fine motor activities. He has also started to return back to his job in a graded manner after 1 year to prolonged physiotherapy treatment at the Centre. His treatment is continuing.
